The article describes a case of anti-Ma2 associated paraneoplastic myeloradiculopathy in a 46-year-old man. The patient had a history of testicular cancer whose neurological syndrome stabilised and anti-Ma2 titres fell after orchidectomy of a microscopically normal testis. He presented with weakness of neck flexion, head drop, and wasting of both forearms and intrinsic hand muscles.
